Connifers Care provides specialist residential care home in the community for people recovering from severe and enduring mental illness.
Our aim is to work in partnership with our service users, professionals, and carers, to provide care and support to this client group. Our approach is to assess needs, develop care plans in collaboration with our client and their team to meet these needs in a safe and clean environment. We use a very structured rehabilitation approach which focuses on developing our service users’ skills in areas such as budgeting, shopping, cleaning and cooking. The emphasis is also on ensuring that our service users are equipped with skills needed to lead a reasonable life in the community, such as attending college, work and employment training, voluntary work schemes. We also create a social setting which is appealing to our residents which also offers opportunities to socialize with others. Recreational activities include trips to cinema, bowling, swimming trips, gym sessions and much more.
Our services are for people who have endured a severe mental illness and are in the recovery phase. In particular, people who have been diagnosed to be suffering from:
- Schizophrenia
- Dual Diagnosis
- Bipolar Affective Disorder.
Our staff team are well trained and experienced in dealing with people with complex and challenging needs.
Our services are geared towards meeting the requirements as set out under Section 117 of the Mental Health Act 1983 – After Care service provision. We are also experienced in the implementation of Guardianship Orders, Section 25 and Section 17. In addition, we have the capability and expertise to manage service users on Conditional Discharge from Section 37/41 after time spent in Medium or High Secure services.
